Background Check Instructions in Minnesota
After submitting your applications to take the NCLEX, you will receive an email with the packet necessary to complete your background check (Subject of the email: "Information about your CBC for Minnesota License”). Once you receive that email (1-2 business days after submitting your application to take the NCLEX), follow the directions below.
If going to get fingerprinted at the BCA, you only need to bring form three (Note: You have to mail forms 1-3 and the PAS Certification Form, so save some time and print them all at the same time). DO NOT sign and date until they tell you to do so. Fill it out in black ink. Bring $10 cash, check, or money order for the fingerprint card. Bring the same ID you used to fill out the form.

Note: Allot at least 30-45 mins into your schedule for this appointment. Also, there are signs posted mentioning restricted/private parking. Be sure to follow the signs for the public parking, which is to the left immediately after turning off of the street.

You can also contact your local law enforcement office (city/town police or county sheriff) to see if they offer fingerprinting there as well. They tend to be less busy and more accessible.

Now it’s time to mail everything.
Return address should read:
Your name
Your street
Your city, state, zip
MN Board of Nursing
The address listed under the instructions for mailing in your fingerprints is incorrect.

Because it is not recommended to bend the fingerprint card, you may want to get a document envelope from USPS. If you don’t know what that is, ask an employee and they’ll point you in the right direction.
